What Each Engagement Typically Includes
PathfinderMD services are built around the same core work, but the way time is used depends on the client’s situation. Some clients need more time spent in conversation, while others may benefit from more behind-the-scenes work, such as deeper chart review, organizing records, preparing written summaries, or communicating with treating clinicians when appropriate.
The package structure provides a starting framework, but time may be adjusted based on what will be most useful for the client. In some situations, time typically set aside for visits may be used instead for record review, care-team communication, appointment preparation, or other focused work related to the client’s medical story.

Review medical records, visit notes, discharge paperwork, medication lists, and test results
Build a working summary of the client’s medical story
Organize diagnoses, symptoms, medications, clinicians, and open questions
Identify gaps, inconsistencies, or unclear next steps in the care plan
Create a prioritized list of concerns to address
Prepare a focused question list for upcoming appointments
Map the current care team, including clinicians, specialists, facilities, and support services
Communicate with treating clinicians when appropriate
Help determine which issues need attention now and which can be monitored
Create practical summaries or planning documents for clients, families, or treating clinicians
