Meet Dr. Wrem
Dr. Wrem created PathfinderMD because he has seen how often families are left without the time, context, or support they need to make thoughtful, person-centered medical decisions.
During more than a decade practicing family medicine, he cared for many patients whose situations were medically complex — multiple diagnoses, multiple specialists, and difficult decisions that affected not only the patient but the entire family. He often saw how hard it could be for people to piece together the bigger picture while navigating a healthcare system that moves quickly and rarely has time to slow down.
That perspective is also personal for him. He has experienced the healthcare system as a patient himself while managing complex health conditions. He knows how vulnerable, confusing, and frightening those moments can feel when the path forward isn’t clear.
Those experiences deepened his commitment to helping families feel heard, supported, and understood when the stakes are high.

J. Alex Wrem, MD is a board-certified family medicine physician with experience caring for people with complex medical and social needs.
Before founding PathfinderMD, Dr. Wrem worked in a large academic health system, where he saw firsthand how confusing and fragmented healthcare can become for clients and families. His background includes primary care, health system leadership, and years of work helping clinicians make better use of medical information.
Dr. Wrem brings a rare combination of clinical experience, health system knowledge, and comfort working through complex medical records. This helps him understand not only what the chart says, but what it may leave unclear: the unanswered questions, the communication gaps, the competing recommendations, and the next steps that families are often left to figure out on their own.

Dr. J. Alex Wrem is a family medicine physician who has created a unique advisory practice for caregivers of loved ones living with dementia.
A Chicago native who has called the Philadelphia area home since college—Go Birds—Dr. Wrem brings both medical experience and a teacher’s mindset to his work. Before becoming a physician, he taught middle school science, a background that continues to shape his clear, practical, and compassionate approach.
The part of medicine he values most is the privilege of being present during vulnerable moments in people’s lives and helping guide them toward the peace of mind they deserve. His practice is built around that same purpose: helping caregivers understand what is happening, ask the right questions, and make thoughtful decisions with more confidence.
Outside of medicine, Dr. Wrem is a husband, father, terrier-mix owner, artist, guitarist, 90s alternative fan, and unapologetic lover of dad jokes. He enjoys painting, drawing, photography, and 3D design printing. You can see his creative work on Instagram here.
One of his favorite places is the shore—especially that first summer moment when your toes hit the water where the waves meet the sand.

No. Dr. Wrem does not become your treating physician or primary care doctor. PathfinderMD does not diagnose, prescribe, order tests, or directly manage treatment.
The goal is to help you better understand and work with the medical team you already have.

PathfinderMD may help adults and families at many stages of life. You do not need to be facing a crisis to benefit from more clarity.
The service is especially relevant when care feels complicated, fragmented, or hard to understand. Common situations include recent hospitalizations, new diagnoses, multiple specialists, medication confusion, unclear recommendations, caregiver overwhelm, family decision-making, or feeling unsure what questions to ask at the next appointment.
Some clients reach out during a major transition, such as a hospitalization, new diagnosis, or change in function. Others reach out because care has gradually become more complicated, and they want to feel better prepared before the next step.

Final medical decisions should be made with the clinicians responsible for your care.

No. PathfinderMD is a private-pay service and does not bill insurance.
This allows the work to focus on time, context, preparation, and individualized support — areas that are often difficult to address within standard insurance-based medical visits.

You do not need everything perfectly organized. Helpful materials may include medication lists, recent visit summaries, hospital discharge paperwork, specialist notes, test results, and a list of current clinicians.
It is also helpful to think about what feels most unclear, what decision or appointment is coming next, and what would make you feel more confident moving forward.
